    Name of the product causing the problem
  • Software Release
    See the product/platform specific sections
  • Operating System
    Windows (2000, XP, Vista), z/OS, Unix, Linux, OS/400, Tandem, etc.
  • Problem title
    The title summarizes the key elements of the problem (see the product/platform specific sections for some examples)
  • Problem Description
    Provide a short description by specifying:
    • the operation that produces the error (configuration, start up, transmission, acquisition, compression, encryption, etc.)
    • error codes and description
    • when the problem has occurred (day and time), also specify if was the first time, repetition of occurrence, under which conditions
    • interaction of the product (e.g., SPAZIO 2 D.P.-SPAZIO 2 z/OS, Data Secure Tandem-Data Secure z/OS)
    • description of the operations that have led to the problem
    • any changes to the system from last successful run (hardware and software)
    • any indications useful for reproducing the error in the lab
